My Health Coach and Trestle Tree logos

Professional health coaches provide confidential personalized assistance to help individuals reach their personal health goals. Coaches are available to meet on campus or by phone. They are employees of TrestleTree, an accredited health transformation organization. Meet the health coaches.

  • Develop a fitness plan
  • Deal with stress

My Health Coach Program/TrestleTree offers departments a variety of wellness seminar topics presented onsite. Refer to Wellness Seminars by My Health Coach Program/TrestleTree for a list of available seminar topics and their descriptions as well as information about how to make arrangements.

Diabetes Management Program

The Diabetes Management Program (DMP) offered in partnership with My Health Coach, is available to eligible employees and their dependents who have been diagnosed with type 1 or type 2 diabetes or pre-diabetes. Participants in the program receive a $250 taxable cash incentive and have copays waived for prescription drugs and supplies related to diabetes care that are categorized as generic (Tier 1) or preferred (Tier 2) in the OptumRx formulary. If individuals are prescribed a non-preferred or excluded medication to manage the condition, their doctors must complete a prior authorization (PA) to cover it. Once the PA is approved by OptumRx, individuals must contact the Benefits Team to request a $0 copay.

To qualify, participants need to: 

  • Enroll in health coaching with My Health Coach
  • Provide the Lab Requisition Form, completed by the participant’s doctor, with current biometric screening results (blood work) for cholesterol, blood sugar, etc. to TrestleTree
  • Meet with a health coach in person or telephonically to discuss the lab results
  • Complete a Health Risk Assessment and Participation Agreement Form with TrestleTree

After the third visit with a health coach, participants qualify for a $250 taxable cash incentive and the copay waiver as long as they schedule a fourth visit  and complete all  steps described above. In order to continue to receive the copay waiver, participants must meet at least quarterly with the health coach, regularly monitor glucose levels, and visit a physician at least once annually.

Condition Management Program

The Condition Management Program (CMP) is an incentive program, offered through the My Health Coach program to participants diagnosed with high blood pressure, high cholesterol, or obesity.

Individuals diagnosed with obesity, who are not taking any medications, receive a free gift to assist them in their weight loss journey.

To qualify for the program and continue to receive qualifying medications through the DMP and CMP at $0 copay, participants need to: 

  • Enroll in health coaching with My Health Coach
  • Provide current biometric screening results to TrestleTree as described above
  • Meet with a health coach in person or telephonically to discuss the lab results
  • Complete a Health Risk Assessment and Participation Agreement Form with TrestleTree
Eligibility

Eligibility begins on the first of the month coincident with or next month following the employee’s date of hire. Refer to the Benefits Plan Eligibility Chart. To participate in the DMP or CMP, individuals must be enrolled in a Princeton medical plan.
